Send a CCDA to the Patient Portal

What:

ChartLogic has added the ability to send a CCDA to the patient portal. Patients can view, download, and transmit a CCD from the portal but have not been able to receive a CCDA at a visit level.

Why:

Before the change, the clinical staff was not able to send a CCDA for a specific note/encounter to the patient portal. Now from the note the staff can send a specific CCDA to the patient portal. This function is only available for patients who have set up their portal accounts.

How:

Within the patient note select the CCDA icon, select send to portal button. This will send the note level CCDA to the patient portal.

If the patient has not set up a portal account, the “send to portal” button is grayed out with a message to why the function is unavailable.

When the patient is logged into the portal, the CCDA appears in the Chart Clinical Document Card.

This new card was created to track CCDA’s sent to the patient and manage the CCDs for the patient to view, download, and transmit.

The elements of the card are as follows:

• Created Date- the date the CCDA (note level CCD) was sent to the portal
• Note Type- the type of the note on creation
• Document type- selected when sending to the portal
• Date of Service- The encounter date of the visit
• Viewed- if the patient has opened the CCDA and viewed it
• Downloaded- if the patient has taken the download action
• Transmitted- if the patient has taken the transmit action

Clicking on the CCDA line will open the function options for view, download, and transmit as well as the details of the sent CCDA.
