New clinical note types are now available in ChartLogic when creating a CCDA to meet ONC certification requirements. The ONC has implemented a set of clinical note types that allow the clinical staff to generate a CCDA with comments connected to the specific note type. This will give the consuming body the ability to have text information about each type for transition of care for a patient.
The certification body has implemented note types with the transition of care requirement. Each note type has a specific LOINC code included and text documentation specific to what and why the CCDA is being created.
At the top of the note, click the “clinical document” button and select Continuity of Care Document from the drop-down menu-> use the multi-select option of the kind of type information you would like to be contained in the CCDA. The text included in the CCDA will come from what is documented in the unstructured card within that specific note.

The text details to go in the CCDA will come from what is documented in the unstructured card. The unstructured card can be added on the add cards option in the note header.
This section of the CCDA will contain documented information as well as any loinc code associated with the specific note type.
