Patients can open and complete forms in the Patient Portal. The patient forms user interface has been updated to be easier to use and self-guided.
Making things simple for users was our top priority for this patient forms product. To make things simple we wanted to capitalize on how and where patients access forms. This part of the feature gives patients a quick and effortless way to access forms in the Patient Portal. We also wanted them to have a better way to see and manage their forms.
There are three different methods that can be used by a patient to access their forms, email, text, and Patient Portal. All methods currently lead them to the Patient Portal to complete. Below are examples of these types of methods. For a patient to enter a form they will need a patient portal account. The links will send them to the patient portal login page.

Patient Portal:
Patients can log in to their patient portal to view and complete forms. The forms area gives patients the ability to see all the forms they have completed and new ones that they need to complete. They can also print forms if they need a paper copy. There is a search function to help them filter the view to what is most relevant for them. Forms can be sorted by each of the headers in the table view.
To complete a form, a patient must have a patient portal account.
Once logged into the patient portal, click on the Patient Forms dashboard icon. Then click on any incomplete form.
This will start the form as seen below.
The patient will need to fill in each section with text or selection as indicated by the field.
*Note: All required fields on a form must be completed before it can be submitted.
When the form is complete, the patient will click the submit button. This will send the completed form to the practice for them to be reviewed and data reconciled.
Forms can still be reviewed by the patient from the patient portal. Patients can also save their work if they want to come back later to complete a form by clicking the save button.
A patient can view forms they have completed by opening the patient portal, navigating to the Forms management area. Then click on the form they want to view
