Within the Patient Portal a patient can view, download, and transmit a continuity of care document which contains their chart data. To view and download the continuity of care document, navigate to the patient chart and click on the Clinical Document icon at the top right.
The patient’s clinical document will open.
Now you may edit this document by clicking on the Cog icon, to include a date range and select available sections to display.
To send the clinical document, open Messaging on the left and click on New Message. Next, select to send the document encrypted through our direct messaging system or unencrypted to any email.
On the left, click Activity Log to display all patient clinical documents that has been viewed, downloaded or transmitted.
