Patient Demographics

What:

Patient Demographics data is now in one place with additional information. ChartLogic has created one access point for all the patient demographic data including organizing the data in a better workflow.

Why:

ChartLogic is improving workflows to access and update patient information.

How:

From the patient demographic icon in the patient header, a patient detail tab will open under the patient.

Chartlogic has better organized the patient and demographic information. New cards have been created. The header of all the cards will display the most important information helping the user decide if there is a need to edit. To edit information on any of the card click the edit icon .

Patient Information Card contains the about the patient vital information. The order of the data elements has been updated for a more natural workflow when capturing patient information.

Patient Demographic Card contains the demographic information of the patient as required by government regulations.

Patient Communication Card has been moved here with easy toggles to enable the forms of communication the patient prefers. The details of where the communication will be received are listed on the right side of the card. These can be edited in the patient information card.

A Clinical Information card has been created to capture the care team information for a patient.

Emergency Contact Card, Authorized Representatives, Name and Address History has been moved here with the same information and functionality.

A few elements previously captured in the demographic card have been moved to the patient chart for ease of tracking and capturing them.

Immunization Shareable and Multiple Birth data is now on the chart Immunization card, able to edit the details directly from it.

The patient smoking status has been moved to the Histories and Objectives Card with the ability to edit from the chart card.
