Episode of Care

What: 

ChartLogic is excited to release the ability to create chart elements under an episode of care. The clinical staff can now create a parent type of care to track episodic notes like pregnancy, and chronic care. 

Why: 

Users need the ability to group notes under one care episode.  They have the need to view and track episodes of care in one location. 

How: 

Configuration is required to enable clinical staff to create an episode or care. Users can create episodes and specify specific note types available when adding to the episode. 

Configuring the Episode 

From the left navigator menu select Configuration-> Episodes

The episode card allows you to add or manage episode types.  Name, Description, Active, and Created date display on the card. The check box is used to change the order on the card which displays in the chart.  

To create an episode, click – configurations -> Episode -> Add episode 

• Name the episode and add a description of the episode if needed. 
• Then select the note types (These will be available when adding an episode within the patient chart. These will be the only note types available.) 

Create an Episodic Care in the Chart 

To create an episode for the patient, add the episode card to the chart by selecting episode card in the add card option.  

From the episode card in the chart click Add episode and fill in the information. 

• Description – description of the episode displayed on the episode card (not required) 
• Type – select the episode type from the drop-down menu (list from the configurations)
• Begin Date – the date the episode starts – (required field) 
• End date – indicates the date the episode ended  
• First Exam Date – captures the first date of exam relating to the episode (not required) 
• Accident Date – captures the accident when applicable (not required) 
• LMP – captures the last menstrual period when applicable (not required) 
• Notes – captures additional notes about the episode (not required) 

Adding a note to a new episode 

Once the user has added the episode, the episode appears with the patient note included in the episode.   Selecting add “new note” follows the current note functionality defaulting the episode it is associated with. 

Completing an Episode 

The episode of care can be finished and completed by clicking the finished button ONLY when all notes have been completed within the episode.  The episode will be locked for editing once it is finished.
