Searching for Patients

Searching for patients in the ChartLogic EMR is easy and simple.

How to do a basic search for a patient:

1. Click in search box.
2. Enter search criteria.

Note: Searches are generally done by last name,first name (IE: Smith,Andrea). It does not require you to have complete name information to find the patient, you can use the first 3 characters of both the last name,first name to find the patient. You can also use symbols to search for patients by other methods. The # symbol in conjunction with the chart number will search for patients with that chart number (IE: #65340). The % symbol will search patients with DOB (IE: %04/27/88). The @ symbol will search patients with a particular Home/Cell phone number (IE: @801-365-1800).

Note: If there are multiple results for a patient search, the system will show all of the results in the advanced search window.

How to use the advanced search:

1. Click Patient Search drop-down.
2. Enter in Search Criteria. You can search by Name, Account (chart number), Home Phone, Mobile Phone, SSN, Address, or DOB. You can use any combination of these as well.
3. Click Search.
4. Select the patient and double click to access the patients chart.
