The only information truly needed for a Referring Provider is the name and NPI. However, for the purpose of correspondence, you may also record the Referring Practice and Referring Practice Sites. When a Referring Practice Site is selected, the phone number auto populates in the Referring Provider form. The phone number may also be manually entered.
