Tables: Practice Sites

Practice Sites relate to box 32 on CMS-1500, Form 1500 for “Office” services. Complete a separate Practice Site form for each location. To add a Practice Site, click New, fill in the required fields on the Practice Site form, and save.

If the Site address is the same as the Practice address, check the “Use Practice” box and the address fields will auto populate. If you enter a CLIA Number, it is very important to be sure the CLIA Type MATCHES your CLIA Certificate assigned to this site.
