Tables: Practice

There will only ever be one entry in this table. It represents the overall business associated with the Tax ID Number. MBS sets up the Practice with the required fields filled in. To add or edit information, double-click on the Practice, edit the desired fields, and click Save.

• If TIN Type is EIN, enter the practice name in the Name field.
• If TIN Type is SSN, enter the practice name in the Name field, and fill in the Last Name and First Name fields with the name associated with the SSN.
• Remember to fill in both the Practice (physical) address and the Remit address, located on a second tab (relevant to Box 33 CMS-1500, Form 1500).
• If the Remit is the same as the Practice address, click the “Use Practice” check box in the Remit tab.
