Tables: Facilities

Facilities relate to box 32 on CMS-1500, Form 1500 for non-office services (e.g. Hospital, ASC, SNF, etc.). For each Facility, fill in the required fields and save.

You can search for the Facility’s NPI in one of two ways:

• Search button: Click magnifying glass to search our database, which is updated quarterly. Search by organization name or address (can use partial) and state.
• NPPES Website: If the organization is not in our database, use this button to check the NPPES website directly.
