Medication Reconciliation – Objective 7
About this Objective: The EP who receives a patient from another setting of care or provider of care or believes an encounter is relevant should perform medication reconciliation.
Measure:
The EP who performs medication reconciliation for more than 50% of transitions of care in which the patient is transitioned into the care of the EP.
Exclusion:
Any EP who was not the recipient of any transitions of care during the EHR reporting period.
Report Calculation:
Denominator: Distinct patients who have a patient medication entered under the selected provider ID
Numerator: Distinct patients who have a patient medication that was reconciled through a CDA or CCDA.
Additional Information:
- The electronic exchange of information is not a requirement for medication reconciliation.
