CCDA Reconciliation Screen
To open a patient’s record from Transfer of Care or Referral in the CCDA Reconciler, perform a search in the Direct Message Report, then double click on the message you want to open.
To open a patient’s record from Transfer of Care or Referral in the CCDA Reconciler, perform a search in the Direct Message Report, then double click on the message you want to open.
After the message opens, click “See C-CDA document” to open the referral or transfer of care in the CCDA Reconciliation screen.
The CCDA Reconciliation screen will open and display some basic patient demographics, and the patient’s allergies, medications, and problems. If the patient matches one already in the EHR, then the Patient ID/MRN will be populated. If the Patient ID/MRN is blank, then the patient is not in the system, and you need to type in a Patient ID to have the patient and their data imported into the EHR.
The left column under allergies, medications, and problems displays data in the TOC or Referral CCDA. The right column shows what will be saved to the EHR. If the patient already exists in the EHR, then it will display the current allergies, medications, and problems for the patient.
If all allergies, medications, and problems need to be saved in the EHR, then click “Merge All” at the top of the screen.
If an individual item needs to be added, then select that item and click the blue arrow between the columns.
The following image shows an example of the Reconciliation screen after allergies, medications, and problems are moved to the right-hand column that displays the data to be saved.
Items with a red “X” next to them are going to be added to the patient’s record in the EHR. Click the red “X” to remove them if they were added by mistake.
If the description of that item is in red, and has a strikethrough, it indicates that the item is already stored in the EHR and will not be imported.
Occasionally, you will see what appears to be the same item being added twice, as “1665362 Aspirin” in the image below. This is either due to the CCDA containing the item twice, or because there is data attached to those allergies, medications, and problems that make the records different. In this case, we suggest importing both into the EHR, and reviewing that information in the patient’s record where the complete data will be displayed.
If you want to return to the state when you first opened the CCDA Reconciliation screen, click “Reset” in the top menu.
To save the patient’s merged record to the EHR, click “Save and Close”.
After clicking “Save and Close”, a confirmation window pops up, and you will be taken to the patient’s record.
