CPOE for Medications, Labs, & Radiology – Objective 3

About this Objective: Use computerized provider order entry (CPOE) for medication, laboratory and radiology orders directly entered by any licensed healthcare professional who can enter orders into the medical record per state, local, and professional guidelines.

Measures:

3A: CPOE – More than 60% of medication orders created by the EP during the EHR reporting period are recorded using computerized provider order entry.

3B: CPOE – More than 30 percent of laboratory orders created by the EP during the EHR reporting period are recorded using computerized provider order entry.

3C: CPOE – More than 30 percent of radiology orders created by the EP during the EHR reporting period are recorded using computerized provider order entry.

Exclusions:

3A: CPOE – Any EP who writes fewer than 100 medication orders during the EHR reporting period.

3B: CPOE – Any EP who writes fewer than 100 laboratory orders during the EHR reporting period.

3B: CPOE Alternate – Providers scheduled to be in Stage 1 in 2016 may claim an exclusion for measure 2 (laboratory orders) of the Stage 2 CPOE objective for an EHR reporting period in 2016.

3C: CPOE – Any EP who writes fewer than 100 radiology orders during the EHR reporting period.

3C: CPOE Alternate – Providers scheduled to be in Stage 1 in 2016 may claim an exclusion for measure 3 (radiology orders) of the Stage 2 CPOE objective for an EHR reporting period in 2016.

Report Calculation:
Medications:

Denominator: Number of medications with a start date between the date range with the provider selected that isn’t deleted and for a patient who also has a visit within the date range. (This includes ALL medications.)

Numerator: Number of medications in the denominator that were prescribed. (This does not include Manage Medication Meds)

Labs and Radiology:

Denominator: Count of total orders within the date range that have the report type of “Lab” or “Radiology” and for the provider selected.

Numerator: Count of total orders in the denominator and were completed or done electronically.

Additional Information:
  • Any licensed healthcare professionals and clinical staff credentialed to and with the duties equivalent of a medical assistant, can enter orders into the medical record for purposes of including the order in the numerator for the objective of CPOE if they can originate the order per state, local and professional guidelines. It is up to the provider to determine the proper credentialing, training, and duties of the medical staff entering the orders as long as they fit within the guidelines prescribed. Credentialing for a medical assistant must come from an organization other than the organization employing the medical assistant.
  • An EP must satisfy all three measures for this objective through a combination of meeting the thresholds and exclusions (or both).
  • CPOE is the entry of the order into the patient’s EHR that uses a specific function of CEHRT. CPOE does not otherwise specify how the order is filled or otherwise carried out.

How to meet this Objective:

  • 3A – You will need to prescribe at least 60% of your medications using ePrecribe to meet this measure.
  • 3B & 3C – You will need to use the order entry module to document at least 30% your lab and radiology orders. To learn how to enable and set up this module, Click Here.
