Clinical Quality Measures

Clinical quality measures, or CQMs, are tools that help measure and track the quality of health care services provided by eligible professionals, eligible hospitals and critical access hospitals (CAHs) within our health care system. These measures use data associated with providers’ ability to deliver high-quality care or relate to long term goals for quality health care. CQMs measure many aspects of patient care including:

  • health outcomes
  • clinical processes
  • patient safety
  • efficient use of health care resources
  • care coordination
  • patient engagements
  • population and public health
  • adherence to clinical guidelines

Measuring and reporting CQMs helps to ensure that our health care system is delivering effective, safe, efficient, patient-centered, equitable, and timely care.

∙ CMS153: Chlamydia Screen for Women. Domain: Pop/Pub Health

∙ CMS166: Use of Imaging Studies for Low Back Pain. Domain: Efficient use of Healthcare Resources

∙ CMS165: Controlling High Blood Pressure. Domain: Clinical Process

∙ CMS126: Use of Appropriate Medication for Asthma. Domain: Clinical Process

∙ CMS127: Pneumonia Vaccinations for the Elderly. Domain: Clinical Process

∙ CMS123: Diabetes Foot Exam. Domain: Clinical Process

∙ CMS65: Hypertension: Improve in BP. Domain: Clinical Process

How to achieve CQM numbers using the ChartLogic EMR:

  • You are required to use the ChartLogic superbill to obtain reporting information.

Note: Not using the ChartLogic superbill does not disqualify you from attesting nor being able to gain a successful submission.

  • Enable MAeHC under Configure > Options then you can access the reporting portal under > Reports > Launch CQM Reporting Portal.
  • You must contact ChartLogic to get an CQM reporting Login Info.
