Provide Patient Education – Objective 6

About this Objective: Use clinically relevant information from Certified EHR Technology to identify patient-specific education resources and provide those resources to the patient.

Measure:

Patient-specific education resources identified by Certified EHR Technology are provided to patients for more than 10% of all unique patients with office visits seen by the EP during the EHR reporting period.

Exclusion:

Any EP who has no office visits during the EHR reporting period.

Report Calculation:

Denominator: Distinct Patients who have a visit within the report date range with the provider selected that isn’t a “Message” or “Form”

Numerator: Number of distinct patients in the Denominator that have had Patient Education.

Additional Information:
  • The education resources or materials do not have to be stored within or generated by the certified EHR technology.
  • Unique patients with office visits means that to count in the denominator a patient must be seen by the EP for one or more office visits during the EHR reporting period, but if a patient seen by the EP more than once during the EHR reporting period, the patient only counts once in the denominator.

How to meet this Objective:

  • Education materials given to the patient must be documented on the exam. To do this either Click Patient Education from the left toolbar, or using voice, say “patient education.” Select the materials you provided the patient. These materials can be templates that you built into the EHR or external documentation such as a scanned in image of a pamphlet that you handed the patient. When finished selecting documents you can print if necessary to provide to the patient, or click OK to save and exit the education screen. Any selected education materials will be placed in text in an addendum at the bottom of the exam.
