About this Objective: The MIPS eligible clinician that transitions or refers their patient to another setting of care or health care clinician (1) uses CEHRT to create a summary of care record; and (2) electronically transmits such summary to a receiving health care clinician for at least one transition of care or referral.

Measure:

The MIPS eligible clinician that transitions or refers their patient to another setting of care or health care clinician (1) uses CEHRT to create a summary of care record; and (2) electronically transmits such summary to a receiving health care clinician for at least one transition of care or referral.

Exclusion:

No Exclusions

Report Calculation:

Denominator: Distinct Patients who have had a Referral Sent within the date range of the report.

Numerator: Number of Patients in the denominator that have had a Summary sent electronically via direct email.

Additional Information:

• This exchange may occur before, during or after the EHR reporting period but must take place no earlier than the start of the same calendar year as the EHR reporting period and no later than the date of attestation in order to count in the numerator.
