Configuring Patient History

Most options and security settings require administrator rights. Please contact your system administrator for assistance.

Accessing History Configuration:

Once logged to ChartLogic, from the main screen you will go to:

Configure > Configure History

This will open your Patient History Configuration screen.

History Configuration:

There are History items that are set as defaults in the system. These are can be edited except Smoking.

You can also edit and add an additional ten Custom History items.

 

Searching Patient History items:

When searching History items, all applicaple history options will show. From here you can add, edit, or delete.

Editing or updating History items

The Yellow Caution symbol indicates that it is not an acceptable MIPS sno-med code.

The Number in the code field indicates that it is an acceptable code and can be reported on.

Highlight and select Edit to select the correct sno-med code from the approved list.

After selecting Edit, an additional window will pop up. It will auto populate what was selected from the invalid snow-med code. Once you select Search, the window will populate a list of acceptable codes to choose from as well as update the system as and any other charts that are currently using the invalid code.

Once the search window updates, checkmark the box with the description that best fits. If it is a common used History item, checkmark the common response box as well. Select OK. This action can be done from both History Configuration or from the Patient History screen.

When first adding and editing codes for a History field, if there are no History items marked as a common response nothing will show until you search for it.

Without performing a search, if there are common responses in the selected History field it will auto populate those common responses.

  

Adding Codes:

You can add History items by selecting the (+) plus sign. An additional window will come up and give you the options to search for codes or add a Custom History item.

From the add pop up window you can search default items, add custom codes, add free texted descriptions and select it if will be a common response. From this screen you can also select multiple codes to add to the Patient History item.

Deleting History items:

Items can be deleted by selecting the red (X). You can select multiple items by holding down the CTRL button and dragging your mouse down through the list or hold the CTRL button on keyboard and select individual codes then clicking the red (X).

After selecting the red (X) to delete, a box will pop up giving a list of all the codes that were deleted. It will also you the option to copy the text in case the delete was done in error so you can add it back into the system or if you need to keep for your records that the code was removed.

 

Accessing Patient History:

From the main screen in ChartLogic select Patient > History.

 

Adding Codes to Patient History:

Multiple History items can be open at once. Note here the item highlighted is displayed on the top above the codes as well.

Multiple items can be selected and move by clicking and dragging your mouse or holding the CTRL button on you keyboard and selecting individual codes. You can move them all at once by using the arrows in the middle of the screen.

After using the arrows to move the History items over, they no longer show in the right column and show in the appropriate History section on the opposite side.

Add, Edit and Delete all work the same here as they do in the Configure Patient History.

Select the (+) to add items

Select the red (X) to delete items.

Select the pencil to edit items.

 

Allergies:

Allergies are still managed in the e-Prescribe section of ChartLogic.
